Occupational Therapy
Trains individuals in self-care skills (training in the use of adaptive equipment, compensatory techniques, and environmental modifications) and behavioral and mental health issues. Sets up goal for Residents plan of care. Supervises COTA when assigned.
Physical Therapy
In a SNF focuses on maintaining, promoting, or restoring lost physical function following a serious injury or illness. Sets up goals for Residents plan of care. Supervises PTA when assigned.
Speech Therapy
Assesses and treats Residents with a wide variety of deficits including, but not limited to, dysphagia, cognition, speech and/or communication difficulties. Sets up goals for Residents plan of care.
COTA
Trains individuals in self-care skills (training in the use of adaptive equipment, compensatory techniques, and environmental modifications) and behavioral and mental health issues. Follows the plan of care set up by the Occupational Therapist to assist Resident to meet their goals.
PTA
Focuses on maintaining, promoting, or restoring lost physical function following a serious injury or illness. Follows the plan of care set up by the Physical Therapist to assist Resident to meet their goals.
Registered Nurse
Must possess encumbered Registered Nurse Licensure either for Missouri or Multi State. Responsible for day-to-day care of Residents. Following Physician orders and implement, document any issues or concerns. Complete assessments as directed. Oversee and direct staff providing direct and non-direct care.
Licensed Practical Nurse
Must possess encumbered Licensed Practical Nurse Licensure either for Missouri or Multi State. Responsible for day-to-day care of Residents. Following Physician orders and implement, document any issues or concerns. Complete assessments as directed. Oversee and direct staff providing direct and non-direct care.
Certified Nurse’s Assistant
Must possess Missouri Certified Nurse’s Assistant Certification through TMU. Responsible for providing day-to-day cares of Activities of Daily Living. Alert Charge Nurse of any refusals of care or concerns.
Food Service Assistant
Responsible for providing assistance in all food services functions as directed/instructed by the Food Service Director and/or designated supervisor (i.e. cook) in accordance with established policies and procedures, applicable federal, state and local stands, guidelines, regulations to ensure quality food service is provided at all times. Must possess a county food handlers’ certification within 30 days of employment.
Infection Preventionist/Clinical Educator
Must possess encumbered Registered Nurse Licensure either for Missouri or Multi State. Responsible for Monitoring, Tracking and Documenting of any infections. Providing education and documenting all educational training with staff.
Social Services Director
Responsibilities include complete required assessment according to State and Federal statutes. Monitor Residents Social, Mental and Physical Needs. Assist Residents with scheduling appointments needed for counseling. Assisting the interdisciplinary team with any care plan associated intervention to increase the outcome of the goal. Work with family and Residents with scheduling Care Conference in accordance with regulations. Assist other team members by participating in various teams to promote teamwork for the facility, Residents and families. Provide tours to families if Admissions Coordinator in not available.
*Minimum of a Bachelor’s degree (BSW/MSW) in social work; OR Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling, or special education; AND One year in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals
